Presenters: Anne Huml, MD, Sarah Cox, MSW, LISW, and Kate Greenway, BS
Biographies: Dr. Huml is a practicing nephrologist at University Hospitals Cleveland Medical Center in Cleveland, Ohio. She also serves as a researcher at Case Western Reserve University's Center for Reducing Health Disparity in Cleveland, Ohio.
Ms. Cox is a social worker at DaVita in Columbus, Ohio.
Ms. Greenway is a patient navigator in Columbus, Ohio.
Presentation Objectives:
- Discuss how the mechanisms of transplant (seven steps toward transplant waitlist) were identified and the common barriers encountered at each step.
- Describe the role of a patient navigator and how this model can be used to create peer-to-peer patient engagement activities to assist patients in working through the process for transplant waitlist.
- List interventions to overcome barriers with a peer-to-peer approach or hemodialysis unit assistance.
- Discuss the value added in utilizing a patient to help other patients in getting on the waitlist.
