This change package is intended to support dialysis facilities and End State Renal Disease (ESRD) Networks in increasing the number of patients using home dialysis modalities, which include peritoneal dialysis (PD) and home hemodialysis (HHD). The change package includes actionable change ideas, collected from top-performing dialysis facilities that have increased the use of home dialysis. The change ideas presented are intended as a menu of interventions from which program leaders can choose to implement within their facilities.

Nephrologist Roundtable

Regional Roundtables provide an opportunity for nephrologists who are interested in implementing either a Transitional Care Unit (TCU) or Telehealth program for Home Dialysis (HD) patients to learn and glean effective practices from other nephrologists and other practitioners who have implemented or sustained a TCU or Telehealth program.

Home Dialysis Heroes

A Home Dialysis Patient Peer Program encourages patients already doing home dialysis to share their “I did it and you can too” mindset to inspire more referrals to a home treatment option. ‚Äč

Universal Staff Education

The Universal Staff Education modules are designed to improve awareness and understanding around home dialysis options. To access the courses, please complete and submit the information on this webpage.

About the Change Package

This change package is intended to support dialysis facilities and End State Renal Disease (ESRD) Networks in increasing the number of patients referred to transplant centers, evaluated for kidney transplantation, and placed on transplant waitlists. The change package includes actionable change ideas, collected from top-performing dialysis facilities related to patients’ being placed on transplant waitlists. The change ideas are intended as a menu of interventions from which leaders can choose to implement within their facilities.

Acts a centralized website for patients for trusted information on each step of the kidney transplant process.

Patient Education Video

Delivers easy-to-understand education directed to patients and is intentional in promotion of the possibilities of receiving a high KDPI or increased risk kidney transplant.

Transplant Trailblazer Program

Provides short-term peer support to dialysis patients from a transplanted patient with the goal of the dialysis peer achieving active transplant waitlist status.
